Please find attached the Participant Declaration, Consent & Health Clearance Form for the 9th National Finswimming Championship 2026, Goa.
Kindly print this form, fill it carefully, and bring the signed hard copy with you on the reporting day at the venue.
Also, please attach a print copy of the Approval / Registration Confirmation Email that you received from our system at the time of registration.
Important documents to carry:
• Printed & signed Declaration Form
• Registration Approval Email print copy
• Original Aadhar Card
• Medical Fitness Certificate
